The Continuity Desk is meant to get better through use. Send corrections, missing fields, accessibility problems, print-layout issues, or field suggestions through Signal. The website does not accept feedback bodies through forms, query strings, email, or phone.
Before transmitting
Send the smallest useful amount of information.
- Start with the form ID, such as CD-05 or CD-16.
- Describe the exact problem, missing field, confusing wording, or print/accessibility issue.
- If suggesting a new form, explain the continuity function it would serve and who would use it.
- Separate direct field experience from speculation about what might happen.
- Do not send completed forms, identification documents, medical records, passwords, credentials, classified or stolen material, or unnecessary third-party personal information.
- Do not send sensitive infrastructure details that should not be public.
Submission does not mean a suggestion will be adopted or endorsed. Signal is a third-party service; no communication channel should be treated as eliminating every privacy or security risk.
